About Urinary Incontinence Treatment and Surgery

Urinary incontinence is common in women, but it is often delayed in being mentioned. The type of complaint directly determines the treatment: leaks that occur during coughing, laughing, or sneezing are based on different mechanisms than leaks that come with a sudden urge. Evaluation includes history, physical examination, urinalysis, and advanced tests if necessary. Treatment may start with pelvic floor exercises and bladder training; if these are insufficient, medical treatment or surgical options such as sling surgery are discussed. The same method is not applied to every patient, and results vary from person to person; expectations are clearly shared during the consultation.

What Is Urinary Incontinence?

Urinary incontinence is a common health issue that can affect women, men, and children of all ages in the community. It is not correct to consider incontinence as normal or accept it as an expected result of aging. Viewing this problem as something to be endured delays treatment. However, with appropriate treatment, it is possible to reduce incontinence and, in many cases, eliminate it entirely.

Urinary incontinence affects millions of women. The most common types in women are urge and stress incontinence.

Types of Urinary Incontinence

  • Urge type: incontinence that occurs with a sudden and uncontrollable urge to urinate, often before reaching the bathroom. Common triggers include opening the door when entering home, going outside in the cold, flushing the toilet, or washing hands.
  • Stress type: incontinence that happens during situations where intra-abdominal pressure suddenly increases, such as coughing, sneezing, straining, or laughing. It is especially common in women who have given birth.
  • Mixed type: a combination of urge and stress incontinence.

How Is the Diagnosis Made?

Patients often hesitate to express this complaint; however, determining the type of incontinence is the first step in treatment. Evaluation includes urine tests, a voiding diary that shows the course of incontinence in daily life, and urodynamic studies when necessary. Detailed questioning is done about when, under what circumstances, and how frequently the symptoms occur.

Treatment for Urge Type

  • Avoiding caffeinated drinks, carbonated beverages, alcohol, spicy and acidic foods,
  • Bladder training techniques aimed at increasing bladder capacity,
  • Pelvic floor muscle exercises,
  • Medication when deemed appropriate.

Treatment for Stress Type

Surgical treatment is prominent in stress incontinence. Especially sling surgeries are the most experienced and commonly preferred method; a mesh is placed under the urethra to provide support and prevent urine leakage when intra-abdominal pressure increases. Before deciding on surgery, the type of complaint is clarified, any accompanying prolapse is evaluated, and the contribution of pelvic floor exercises is reviewed. The expected benefits and possible risks of the procedure are discussed in detail before surgery.

Helpful Habits in Daily Life

Daily habits play a significant role in the outcome of treatment. Distributing fluid intake evenly throughout the day, preventing constipation, managing weight, and regularly continuing pelvic floor exercises can contribute to reducing symptoms. Performing exercises correctly with the right muscles and consistently is more important than the number of repetitions. Keeping a voiding diary noting when and under what circumstances symptoms occur facilitates both diagnosis and evaluation of treatment response.
