About Menstrual Irregularities and Abnormal Uterine Bleeding

Irregular menstruation is not a single disease but a symptom that can have various underlying causes. Hormonal imbalances, thyroid problems, polycystic ovary syndrome, fibroids and polyps, as well as certain medications, are among the most common causes. Evaluation involves using a menstrual calendar, physical examination, ultrasound, and hormone tests when necessary; sampling from the uterine lining may be considered in women over forty and those at risk. Treatment varies depending on the cause and ranges from medication to interventional methods. If the bleeding is heavy enough to affect daily life, evaluation should not be delayed.

What Is Menstrual Irregularity?

The menstrual cycle is a process directly related to women's health and fertility, typically recurring every 28 days on average. During this process, a series of changes occur in the body, with hormones playing a significant role in these changes. Many gynecological and metabolic diseases can disrupt the cycle.

Menstrual irregularity is defined when menstrual bleeding occurs more frequently than every 21 days or less frequently than every 35 days. For example, having a period every 20 days or more often, or every 36 days or less often, falls within this definition. Individuals whose cycles do not repeat regularly may have an underlying health issue.

What Causes Menstrual Irregularity?

Irregularity can be related to many gynecological diseases as well as some metabolic disorders and psychological conditions. The most common causes include:

  • Uterine polyps and fibroids,
  • Thickening of the uterine lining,
  • Cervical, uterine, and ovarian cancers,
  • Diabetes,
  • Anxiety disorders, stress, and depression,
  • Use of certain hormonal medications.

Diseases beyond these categories can also affect the cycle. For a definitive diagnosis, it is essential to consult a specialist in obstetrics and gynecology.

What Should You Pay Attention To?

Menstrual irregularity often appears as a symptom of another disease; therefore, it does not have a unique symptom of its own. However, the following criteria can help recognize the condition:

  • Having periods more frequently than every 21 days,
  • Having periods less frequently than every 36 days,
  • Not having a period for more than three months,
  • Bleeding lasting less than two days or more than seven days.

How Is the Evaluation Done?

During the examination, the onset of the complaint, cycle intervals, amount and duration of bleeding are discussed in detail. Gynecological examination and ultrasound are used to assess the uterus and ovaries; hormone and blood tests are requested if deemed necessary. The goal is to identify the cause behind the irregularity, as treatment is tailored accordingly. Ignoring abnormal bleeding for a long time can lead to anemia and delay the detection of the underlying condition. Therefore, individuals experiencing persistent changes in their cycle are advised not to postpone evaluation.

When Should You Not Delay?

Excessively heavy bleeding, spotting between periods, and bleeding after intercourse are situations that require prompt evaluation. Considering that cervical, uterine, and ovarian cancers are among the possible causes, it is important not to delay addressing these symptoms. If fatigue, palpitations, and shortness of breath accompany the condition, anemia should also be investigated. Noting the start and end dates of the last few cycles before the appointment helps provide a clearer understanding of the situation.
