About Fibroid Treatment and Follow-up
What is a Fibroid?
Fibroids are masses that develop in the uterine wall and are mostly benign. They are common in women of reproductive age and are influenced by hormones, especially estrogen. Whether a fibroid causes symptoms depends on its size, number, and location within the uterus. Some women may have small fibroids that go unnoticed for years, while others may experience large fibroids that cause symptoms interfering with daily life.
What Are the Symptoms of Fibroids?
Symptoms vary depending on the size, number, and location of the mass. The most common symptoms include:
- Painful and heavy menstrual bleeding: fibroids pressing on the inner surface of the uterus can increase pain and bleeding during menstruation.
- Pressure and fullness in the lower abdomen: growing fibroids can create pressure in the abdominal area, sometimes turning into prolonged abdominal pain.
- Frequent urination: fibroids located near the bladder can increase the need to urinate.
- Constipation: fibroids pressing on the intestines can cause digestive complaints.
- Difficulty conceiving: depending on their location and size, some fibroids may contribute to infertility issues.
How is a Fibroid Diagnosed?
Evaluation begins with a gynecological examination; during the exam, the size of the uterus and any abnormalities are assessed. Imaging methods are used to clarify the location and size of the fibroid:
- Ultrasound: the most commonly used method to show the presence, size, and location of fibroids.
- Magnetic Resonance Imaging (MRI): provides more detailed images and helps clearly identify the fibroid's position within the uterus.
- Hysteroscopy: allows direct visualization of the inside of the uterus and is usually planned alongside other examinations.
Why Do Fibroids Develop?
The exact mechanism by which fibroids develop is not fully known. However, some factors thought to play a role include:
- Hormone-containing medications such as birth control pills used irregularly without medical supervision,
- Excess weight and obesity,
- Hormonal imbalances that may occur during menopause.
Understanding the extent to which these factors affect your situation is only possible through a detailed examination.
Monitoring and Treatment Approach
Not all fibroids follow the same course; therefore, decisions are made by looking at the overall picture rather than a single criterion. The size, number, and location of the fibroid, how much the symptoms affect daily life, your age, and pregnancy plans are all evaluated together. For fibroids that do not cause symptoms, regular monitoring with ultrasound at intervals may be sufficient; when bleeding and pressure symptoms become significant, treatment options are considered. During the examination, the recommended option, its expected benefits, and possible risks are explained in simple terms, and the decision is made together.
When Should You Seek Medical Attention?
A significant increase in menstrual bleeding, progressively worsening pain during periods, a feeling of fullness or pressure in the abdomen, frequent urination, and persistent constipation are sufficient reasons for evaluation. Since prolonged heavy bleeding can lead to anemia, this condition should not be considered normal or something to get used to. It is also beneficial for individuals who have been diagnosed with fibroids and are planning pregnancy to review their current condition before attempting to conceive. Bringing your previous ultrasound reports, a list of medications you are taking, and your last menstrual period date to the examination will facilitate the evaluation.
